The Client Year-end Reporting Setup Form is required each year by GOLDPoint Systems to provide year-end tax information to the IRS concerning any accounts or vendors that qualify for year-end tax reporting. This form is required as part of GOLDPoint System’s SOC audit to ensure we are providing you and your customers with timely IRS information.
We have made this form as easy as possible to fill out using PDF-fillable forms and electronic signatures. Even if you do not anticipate having any qualifying accounts/vendors or you do not use GOLDPoint Systems to process your year-end taxes on behalf of your account owners, we need your institution to complete this form for our records.
If you are not using us to report information to the IRS on behalf of your account owners and vendors, we still need you to complete the Contact Person and designate that you will not be using us to report year-end. Then sign the form.

These instructions will help you better understand how to fill out the tax reporting form. Contact Persons
Please fill out the contact person of the person at your institution who we can contact if any problems arise during IRS year-end processing. We may need to contact that person at odd hours during the night when year-end is processing.
You only need to fill out one contact person on the form if your institution does not use GOLDPoint Systems to process IRS year-end.
At the end of the document we need a signature from someone at your institution verifying they have read the form and indicated any applicable fields.
