Availability

This statement can be run in October every year as a test and on December 31 of every year. This report can be printed to paper and/or GOLDView as per institution request.

 

Purpose

This is a combined statement for all retirement account statements in a retirement plan. It is sorted by short name or tax identification number (TIN).

 

As of December 2001, this form will not be updated as regulations change. It is considered obsolete since 5498 forms are required as of December 31 of each year. This statement was used prior to 2002 to meet the requirement of the fair market value statement. Be aware that this statement may not include all transaction types.
